WHAT TO EXPECT AND WHAT YOU’LL NEED
BEFORE YOUR SCAN
To prepare for your Live Scan appointment, you will need:
1. Live Scan form provided by your employer or licensing board (3 completed copies)
2. Social Security Number
3. DOJ Approved Photo ID
DURING YOUR SCAN
In order to receive Live Scan service you must present a valid photo ID to the Live Scan technician. The following are DOJ-approved forms of ID:
Passport, Valid Driver’s License, California DMV Card, Alien Registration/Immigration Green Card, Military ID Card, Mexican Consulate ID Card (Matricula Consular de Alta Seguridad)
